Is there a standard length for lanyard-style eyeglass holders, either with a central loop or with the earpiece loops?

A fairly standard lanyard would be 36 to 38 inches long, depending upon the wearer. It's really a comfort issue, with such a wide range of human heights. We recommend using a length of ribbon, string, yarn or whatever you have on hand, adjusting it around the intending wearer's neck to find a comfortable length. Simply cut your lanyard material to that length (plus extra for knots, loops or other fastening method), and you're good to go!
